An *annual *Health and Medical Record (Parts A & B –

is *applies to all participants*. Please check with Diane Gill or Martina
Bishopp, Activities Coordinators, to ensure your records are update to date.

*For 2019 Camp Ingersoll participants, scouts and adults,* Parts A & B PLUS
Part C Pre-Participation Physical

to be completed by a certified and licensed physician (MD, DO, nurse
practitioner, or physician’s assistant is required and due to our
Activities Coordinators *by Monday, June 10th*. *Please also include a
copy of the front and back of your health insurance card. It is also
helpful to provide 2 copies of all pages, because 1 copy will be given to
the camp’s medical staff. *

*For 2019 Philmont Trek participants, scouts and adults, *complete all
parts of this specific *FORM

*which includes Part C Pre-Participation Physical, to be completed by a
certified and licensed physician (MD, DO, nurse practitioner, or
physician’s assistant is required and due to our Activities Coordinators *by
